The home care RN is a dynamic professional clinician focused on population management with demonstrated capability to provide relationship-based care within a multidisciplinary team. The home care RN utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care for a variety of complex patients in collaboration with patients, families, physicians, and the Continuum therapy staff. Continuum's goal is to educate and empower patients and families and achieve exceptional patient outcomes. MINIMUM REQUIREMENTS – Registered Nurse (RN), Home Health – $15,000 Sign-On Bonus:
PHYSICAL DEMANDS – Registered Nurse (RN), Home Health – $15,000 Sign-On Bonus:
Education: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.
Experience: 1 year of relevant experience.
Licensure: Licensed to practice as a registered nurse in the Commonwealth of Virginia required. Valid Virginia driver's license required. American Heart Association (AHA) Healthcare Provider BLS certification required.
Job may require sitting for prolonged periods, frequently standing, traveling, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 20–50 lbs. May be exposed to chemicals, blood/body fluids, and infectious disease.
