The University of Virginia's Department of Pediatrics, Division of Neurodevelopmental Behavioral Pediatrics, seeks candidates for a Patient Family Navigator opening. The incumbent will work with patients and families to provide support and promote pediatric mental healthcare coordination and system navigation between the UVA Health System and Virginia Mental Health Access Program (VMAP), community agencies, and resources.

Along with collaborative work with their regional VMAP HUB team, responsibilities will include maintenance of up-to-date resources for local child mental health services, family support services, community resources, and their availability across the pediatric and adolescent age ranges; providing care coordination and resource navigation support services for Primary Care Providers (PCP), including feedback to PCPs on family engagement and services; assisting with planning and implementation of patient and family education and support activities, workshops, and training; assisting with problem-solving of complex patient care issues; and developing and maintaining relationships with local intake coordinators at community mental health agencies, clinics, and organizations.

The successful candidate will be expected to:
  • Provide support and assistance to patients and families during the intake process (e.g., assistance with completing questionnaires and intake forms). Assist patients and families in being proactive regarding their healthcare.
  • Assist in learning to communicate effectively with healthcare educators and other service providers.
  • Assist patients and families in identifying and prioritizing their needs.
  • Provide care coordination and resource navigation services using a patient- and family-centered approach.
  • Identify and address service gaps and barriers that make it difficult for patients and families to meet their healthcare needs.
  • Communicate with the clinical team regarding status of referrals, outreach, and care coordination efforts.
  • Compile and maintain up-to-date lists of local and statewide resources as appropriate.
  • Collect, update, and/or create resource materials to foster understanding of patient care needs.
  • Develop and maintain ongoing connections with community resources, support groups, and other service providers.
  • Collaborate with community partners to share current resource information and advocacy approaches.
  • Facilitate patient enrollment and participation in any research or other relevant healthcare initiatives as appropriate.
  • Assist with planning and implementation of patient and family education and support activities, workshops, and trainings. Assist with problem-solving of complex patient care issues.
  • Investigate, document, and resolve or refer as appropriate problems and conflicts.
  • Constantly and consistently evaluate processes, identify opportunities for improvement, and offer constructive ideas and solutions. Actively participate in solutions and implementation.
  • In addition to the above job responsibilities, other duties may be assigned.

Qualified candidates must have:
  • Education: Bachelor's degree.
  • Experience: Two years of progressive work experience, including experience working with complex patient populations, medical terminology, and customer service.

Required knowledge, skills, and abilities:
  • Knowledge of mental health resources and referral options available in our region.
  • Strong oral, written, and interpersonal communication skills.
  • Strong organizational skills and attention to detail.
  • The ability to work independently and as part of a team.

Preferred qualifications and experience:
  • Experience navigating healthcare systems and/or mental health services.

Physical demands:
This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings and programs.

This position is located in Charlottesville, VA and will have the option for remote work. Review of applications will begin on September 23, 2022, but the position will remain open until filled. The anticipated hiring range is $45,000-$48,000 annually, commensurate with education and experience.

The University will perform background checks on all new hires prior to employment. A completed pre-employment health screen and drug screening are required for this position prior to employment.
