Director, UVA Continuum Home Health

This position directs and is responsible for the functions of UVA Health System's Continuum Home Healthcare in accordance with Health System policies, procedures and standards. Directs the activities of clinical care provided by an interdisciplinary team of nurses, therapists (PT, OT, SLP), home health aides, medical social workers and administrative staff. Ensures the enhancement of business development of the assigned department. Drives continuous improvement and fiscal success. Requires an in-depth knowledge of the home health regulatory requirements. Responsibilities include effective resource utilization; financial management; customer service; maintenance and development of policies and procedures; staff education/professional development; and continuous quality improvement using lean methodologies.

  • Accountable for the agencies compliance with all regulations, laws, policies and procedures. Educates all staff members about state, federal and accreditation requirements (as applicable). Maintains compliance with all local, state and federal laws regarding licensure and certification of personnel.
  • Applies understanding of Medicare (OASIS/PDGM) and other payment models to optimize claim management.
  • Collaborates to assist with Health System initiatives (LOS Reduction, Bundled Payments, Decreased Hospital Readmissions).
  • Ensures that all services are billed as provided to the proper payer source. Analyzes available reimbursement data, identifies trends and initiates appropriate plans of action. Collaborates with appropriate staff to complete billing adjustments and maintenance of accurate records of these adjustments.
  • Directs management team members with appropriate assignments of responsibilities to support agency team members and support other members of the medical community to promote home health services.
  • Meets budgeted admission goals in collaboration with Business Development team.
  • Provides input, guidance, and feedback related to marketing activities and agency marketing plan to ensure that goals for growth are met. Participates in marketing activities.
  • Oversees staff recruitment, hiring, assignment, evaluation and guidance to ensure employment of qualified staff. Ensures that the agency adheres to the appropriate staffing and productivity standards.
  • Ensures adequate orientation of every team member assigned to the department.
  • Assigns and directs staff with appropriate skills to perform duties. Coaches, directs and encourages staff through use of formal and informal means. Provides leadership planning for staff education programs, administrative/staff meetings, in-services, and encourages continuous education and staff development for all team members. Sets positive attainable expectations, objectives and goals for the department.
  • Oversees necessary employee corrective action and discipline issues objectively, in consultation with HR.
  • Uses knowledge, experience, and other resources, as necessary, to make logical decisions, solve problems, analyze work processes and make suggestions for improvement.
  • Develops department budget in coordination with the administrator.
  • Responsible for the department's clinical performance along with delivery on key metrics within established budget.
  • Ensures implementation of a performance improvement program to drive quality care, positive patient outcomes and address negative trends. Ensures issues and deficiencies identified during internal or external surveys or audits are addressed and corrected.
  • Collaborates with the area referral sources in implementation of clinical operations standards and processes.
  • Participates in the on-call process, including admin on-call, preparation of the on-call schedule and supports on-call operations.
  • Provides oversight and approval of payroll, to ensure that agency is operating in accordance with current pay practices.
  • Ensures operations are in accordance with work-flow standards.
  • Monitors and evaluates management team performance. Holds employees accountable for the attainment of defined goals and objectives. Executes effective performance redirection when needed. Develops employees through coaching, mentoring, and formal/on-the-job training and development opportunities.
  • Serves as point-of-contact for Home Health to the service area's community, patients, providers, insurers, referral sources, regulatory agencies and other stakeholders.
  • Performs other related duties as assigned.


Position Compensation Range: $100,817.60 - $161,304.00 Annual

  • Education: Bachelor of science degree required. Master of science in related field preferred and required to be obtained within 2 years of hire or transfer.
  • Experience: 2 years of management experience required; 3 years of home health experience required.
  • Licensure: License to practice as a registered nurse, physical therapist, occupational therapist or speech therapist in the Commonwealth of Virginia preferred.


This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings and programs.
