Responsible for provision of Advanced Practice Provider services, including assessment and management of patients with inflammatory bowel disease (IBD) – Crohn's disease, ulcerative colitis, or pouchitis in the ambulatory clinic setting within the scope of licensing, training, and credentialing, dependent on specialty.
Manage outpatient routine and established IBD patients with Crohn's disease, ulcerative colitis, or pouchitis with a focus on chronic care management, monitoring, prevention, and patient education.

Work collaboratively with other medical colleagues and members of the IBD interdisciplinary team.

Provide follow-up office visits for IBD patients who are stable and/or require medication monitoring.

Provide rapid access/same-day appointments for patients with disease exacerbations or complications associated with treatment.

Provide timely follow-up appointment after hospital discharge to help reduce readmissions.

Respond to patient phone calls (advice, refills requests, medication questions, triage, etc.).

Order and interpret labs and tests for monitoring for side effects and response to treatment.

Management and monitoring of medications.

Prescribe refills.

Treat complications of medications, such as infections.

Provide medication teaching.

Serve as a point of contact for IBD patients undergoing infusion therapies at UVA facilities.

Assist patient to develop self-management strategies for dealing with symptoms of flare-ups (e.g., preventing dehydration, when to call clinic, when to go to ER).

Serve as a liaison between the patient and other members of multidisciplinary team involved in the patients care.

Ensure patient has appropriate screening tests and is up to date with health maintenance recommendations.

Monitor the nutritional status of IBD patients and refer to nutrition therapy as needed.

Maintain educational materials for patients, caregivers, and other healthcare professionals.

Provide patients with information, education, and support to empower them to make informed decisions and assume an active role in the decision-making process.

May be involved with clinical trials and research.

Position Compensation Range: $81,078.40 - $129,729.60 Annually
MINIMUM REQUIREMENTS

Education: Graduate of an accredited graduate school of nursing or physician assistant program with a master's degree.

Experience: Department- and unit-specific.

Licensure: License or license eligibility as an Advanced Practice Provider (Nurse Practitioner, Physician Assistant, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to practice as a registered nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies. Current Basic Life Support (BLS) required. Current PALS/NRP, as required specific to position. Advanced Cardiac Life Support (ACLS), as required specific to position.

Job requires standing for prolonged periods, frequently bending/stooping, and reaching (overhead, extensive, and repetitive). Repetitive motion: computer keyboard. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 50-100 lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids, and infectious disease.
