Careers are defined by moments. Moments you decide to pursue excellence. Moments when you grow personally and professionally. And the moment when you take the next step in your career.
Your dedication to patient care, commitment to clinical excellence, pursuit of knowledge and focus on team-based medical care have prepared you for this moment — to join the Transitional and Home Health care teams within one of the nation’s premier academic health systems.
Whether our patients require long-term care in our Transitional Care Hospital or if they are aging in place, UVA Heath’s Transitional and Home Health teams are there to provide advanced patient-focused care to our aging populations when and where they need it most. Utilize the full breadth of your clinical expertise on a team that is dedicated to delivering compassionate care.
Transitional Care Nursing
UVA Transitional Care Hospital is the first long-term acute care hospital (LTAC) in the Charlottesville area and one of only six hospitals of its kind in the state of Virginia. Our freestanding facility, located close to UVA Health’s main campus, allows nurses to provide extended care for patients who are recovering from surgery, weaning off ventilators or have complex medical conditions or wounds that require daily nursing, therapy, nutritional service and a physician. Here, you will collaborate in a team-based environment, gain experience with advanced technologies and procedures, and coordinate closely with clinical experts from a wide range of specialties while helping our patients recover and return home.
Home Care Nursing, Therapy and Allied Health
When advanced care needs to be delivered at home, UVA Health nurses, therapists and allied health professionals are there — bringing advanced care, therapeutics and education that promote our patient’s health, recovery and independence. Our home care teams are some of our most knowledgeable and clinical advanced providers, capable of working with a wide array of patient conditions ranging from acutely ill to chronically disabled to terminally ill. This integrated care team works together to bring academic medicine to our patients at home in the city of Charlottesville, as well as Albemarle, Culpeper, Greene, Madison, Orange, Louisa, Fluvanna, Nelson, Buckingham, Augusta and Rockingham counties.
Join our community of medical practitioners, faculty, students and other team members who learn and even live together to exercise ingenuity, develop self-governance tools and push the boundaries of knowledge in service to the common good.
If you're ready to be part of something greater, this is your moment.
